Please fill out the form below and submit if your child is available to compete in
the meet on December 5-7 at York University

Note: By submitting this form you are confirming that your child is available to
compete in the upcoming meet and you are responsible for any fees incurred if
your child is unable to attend or does not show up (emergencies are the only
exception)

The coaches will determine if your child/ children are ready to compete  based
on their  availability, commitment to  practice, meet preparation and there 2014
registration fees are paid.
